You walk into a clinic in Australia feeling terrible — sore throat, runny nose, the works. You wait. And wait. Then finally, a doctor sees you for about seven minutes, hands you a prescription, and sends you on your way. No detailed explanation. No gentle nod. Just a quick "You'll be right, mate" and you're out the door.
For many Japanese people, this can feel a little... abrupt. Back home, doctors often take more time to explain things carefully, and there's a certain warmth to the whole process. Overseas, especially in countries like Australia, the UK, or the US, the vibe is much more matter-of-fact. The doctor isn't being rude — that's just how it plays out.
Another thing that throws people off: you often can't just rock up to a specialist. You need a referral from a GP first. It sounds like a hassle, but once you get your head around the system, it actually makes sense — GPs act like a first filter.
Oh, and the bill. In Australia, Medicare covers a lot, but not everything. If you go to a bulk-billing clinic, you pay nothing. But plenty of doctors charge a gap fee on top of what Medicare pays back. First-timers often get caught out by this, so it's worth asking upfront: "Do you bulk bill?"
Medical systems are deeply cultural. What feels cold in one country is just efficient in another. Knowing what to expect makes the whole experience a lot less stressful — and hey, at least you'll know what to say at the reception desk.

豆知識（Cultural Tip）
オーストラリアの「バルクビリング（bulk billing）」は、Medicareが診察費を全額負担してくれる仕組みで、患者の自己負担がゼロになります。ドラマ『Offspring』や『Doctor Doctor』でも、GPのクリニックが舞台になるシーンがよく出てきますが、あの気さくな雰囲気がまさにリアルなAU医療体験そのもの。「You'll be right」はオーストラリア版の「大丈夫だよ」で、お医者さんもカジュアルに使うフレーズです。

日本人からすると、これがちょっと……あっさりしすぎに感じるんですよね。日本では丁寧に説明してくれる雰囲気があるけれど、オーストラリアやイギリス、アメリカでは、医療はもっとサクサク進むのが普通。先生が冷たいわけじゃなく、それが「普通の流れ」なんです。
もうひとつ戸惑うのが、専門医への受診方法。いきなりふらっと行くことはできなくて、まずかかりつけ医（GP）に診てもらって、紹介状をもらう必要があります。最初は面倒に感じるけれど、このシステムに慣れてくると、GPが最初の振り分け役として機能しているのがわかってきます。
そして費用の話。オーストラリアではMedicareという公的保険が多くをカバーしてくれますが、全額ではありません。「バルクビリング」対応のクリニックなら自己負担ゼロですが、そうでない場合はMedicareの払い戻し額との差額（ギャップフィー）が発生します。初めての人は思わぬ出費に驚くことも多いので、受付で「バルクビリングですか？」と確認するのがおすすめです。
医療システムって、実はすごく文化的なもの。ある国では「冷たい」と感じることが、別の国では「効率的」なだけだったりします。事前に仕組みを知っておくだけで、海外での病院体験がずっとラクになりますよ。
